Understanding the Thyroid’s Role in Fertility

The thyroid gland produces hormones, primarily thyroxine (T4) and triiodothyronine (T3), that influence nearly every system in the body, including the reproductive system. Thyroid hormones interact closely with the hypothalamic-pituitary-ovarian axis, the hormonal pathway responsible for regulating ovulation and the menstrual cycle. When thyroid hormone levels are too high or too low, this delicate hormonal balance can be disrupted, leading to irregular or absent ovulation, irregular menstrual cycles, and difficulty conceiving.

Hypothyroidism and Fertility

Hypothyroidism, a condition in which the thyroid gland produces insufficient hormone, is the most common thyroid disorder affecting women of reproductive age. Even subclinical hypothyroidism, a milder form in which thyroid-stimulating hormone (TSH) levels are elevated but T4 levels remain within the normal range, has been associated with reduced fertility, higher rates of miscarriage, and complications during pregnancy such as preeclampsia and preterm birth.

Symptoms of hypothyroidism can be subtle and easily attributed to other causes, including fatigue, weight gain, cold intolerance, dry skin, hair thinning, and depression. Hyperthyroidism and Fertility

Thyroid Antibodies and Autoimmune Thyroid Disease

Some women have normal thyroid hormone levels but test positive for thyroid autoantibodies, most commonly thyroid peroxidase (TPO) antibodies. Research has shown that the presence of these antibodies, even with normal thyroid function, is associated with an increased risk of miscarriage and, in some studies, reduced implantation rates during IVF. Patients found to have thyroid antibodies may benefit from closer monitoring throughout pregnancy.

For patients with a personal or family history of thyroid disease, or those with symptoms suggestive of thyroid dysfunction, additional testing and endocrinology referral may be recommended.

For women undergoing IVF, we generally aim to keep TSH levels within an optimal range, typically below 2.5 mIU/L, before beginning treatment, as this range has been associated with better implantation and pregnancy outcomes. If levels fall outside this target, thyroid hormone replacement or antithyroid medication can usually correct the imbalance within a matter of weeks, allowing treatment to proceed once levels stabilize.

Thyroid hormone needs typically increase during pregnancy, and patients with pre-existing thyroid conditions require close monitoring and dose adjustments throughout gestation.
